FCRA Disclosure & Authorization

The federal Fair Credit Reporting Act disclosure and your written authorization for Shipr to obtain background and motor-vehicle reports through a consumer reporting agency to confirm your eligibility to drive.

1. Disclosure Regarding Background Investigation

Shipr LLC ("Shipr," "Company," "we," "our," or "us") may obtain one or more consumer reports and/or investigative consumer reports about you for independent-contractor qualification purposes in connection with your application to become, and if approved your continued participation as, an independent contractor using the Shipr platform (the "Platform").

This Disclosure is provided pursuant to the federal Fair Credit Reporting Act ("FCRA"), 15 U.S.C. §1681 et seq. It is provided to you in a standalone document so that it is clear and conspicuous.

A consumer report may include information regarding your:

  • Identity
  • Social Security Number verification
  • Current and previous addresses
  • Criminal history records
  • Motor Vehicle Records (MVR)
  • Driving history
  • License status
  • Employment verification
  • Education verification (when applicable)
  • Professional license verification
  • Public court records
  • Government sanctions
  • Sex offender registry searches
  • Terrorist watch lists
  • Fraud prevention databases
  • Other lawful background information permitted under applicable federal, state, and local law

We may obtain these reports from one or more consumer reporting agencies ("CRAs"), including but not limited to Checkr, Inc., or another qualified background screening provider.

Investigative consumer reports, if obtained, may include information gathered through personal interviews concerning your character, general reputation, personal characteristics, mode of living, work habits, integrity, reliability, and other information relevant to determining your qualifications to provide services through the Platform. If an investigative consumer report is requested, you have the right to request additional information about the nature and scope of the investigation as required by applicable law.

Consumer reports may be obtained:

  • Before approval as a Shipr Driver;
  • Periodically during your participation on the Platform;
  • Following safety incidents;
  • During investigations involving fraud or misconduct;
  • When required to verify continued eligibility.

We will use any information obtained solely for lawful business purposes related to determining your eligibility to operate on the Platform. Nothing in this Disclosure constitutes an employment contract or guarantees approval as a Shipr Driver.

2. Authorization for Consumer Reports and Background Investigation

By accepting this document, you acknowledge that you have received and carefully read the foregoing Disclosure Regarding Background Investigation.

You authorize Shipr LLC, its subsidiaries, affiliates, contractors, agents, consumer reporting agencies, and authorized representatives to obtain consumer reports and investigative consumer reports concerning you for purposes relating to your application, qualification, onboarding, and continued participation as an independent contractor on the Platform.

You specifically authorize Shipr to obtain, review, and periodically update information relating to:

  • Criminal history records
  • Motor Vehicle Records (MVR)
  • Driver’s license status
  • Driving record
  • Identity verification
  • Address history
  • Employment verification
  • Education verification (if applicable)
  • Professional licenses
  • Government sanctions
  • Public records
  • Other lawful background information permitted by applicable law

You understand that Shipr may obtain additional consumer reports during your relationship with Shipr to evaluate continued eligibility. You authorize all courts, law enforcement agencies, government agencies, educational institutions, employers, licensing authorities, financial institutions, and other persons or entities having information about you to release such information to Shipr or its designated consumer reporting agency.

You authorize electronic copies, photocopies, facsimiles, and digital reproductions of this Authorization to be treated as originals. You certify that all information you provide to Shipr is true, complete, and accurate to the best of your knowledge, and you understand that knowingly providing false information or omitting material information may result in denial of your application or deactivation from the Platform.

3. Continuing Authorization

You understand that this Authorization remains valid throughout your participation as a Shipr Driver unless revoked in writing, except to the extent that applicable law permits continued reliance on this Authorization.

Revoking this Authorization may result in suspension or removal from the Platform if Shipr is unable to verify your continued eligibility.

4. Your Rights Under the Fair Credit Reporting Act

If Shipr intends to take adverse action based in whole or in part on information contained in a consumer report, Shipr will comply with the Fair Credit Reporting Act by providing, as required by law:

  • A Pre-Adverse Action Notice;
  • A copy of the consumer report relied upon;
  • A copy of "A Summary of Your Rights Under the Fair Credit Reporting Act";
  • A reasonable opportunity — typically 7 days — to dispute inaccurate or incomplete information with the consumer reporting agency before any final adverse-action decision is made.

Following completion of the legally required process, Shipr will provide a Final Adverse Action Notice if applicable.

Shipr does not make decisions regarding the accuracy of consumer reports. Any disputes regarding report accuracy must be directed to the consumer reporting agency that prepared the report.

The Driver Independent Contractor Agreement (Section 7) and the Deactivation & Appeals policy describe how these rights fit into the broader onboarding and account-review process.

5. Applicant Certification & Electronic Signature

By accepting this document, you certify that:

  • You have read this Disclosure and Authorization in its entirety;
  • You understand the purpose for which consumer reports may be obtained;
  • You voluntarily authorize Shipr to obtain such reports;
  • You understand your rights under the Fair Credit Reporting Act; and
  • You understand that this Authorization may be executed electronically.

You accept this Disclosure and Authorization electronically when you submit your driver application. Your acceptance serves as your electronic signature, and we record the version you accepted along with the date, time, and device information as your authorization. The personal details required to run the report (such as your date of birth and driver’s license number) are collected securely by our screening provider, Checkr.
